Physiotherapy management for cervical dystonia

Cervical dystonia is a neurological movement disorder that causes involuntary muscle contractions in the neck. These contractions can lead to abnormal head or neck positions, muscle tightness, pain, tremor, reduced movement and difficulty with everyday activities such as driving, working, reading or socialising.

While cervical dystonia can be complex, the right combination of medical management and physiotherapy can make a meaningful difference.

At Coffs Neuro Physio, we support people with cervical dystonia through targeted physiotherapy, education and rehabilitation. We also collaborate with Neurologists who deliver Botulinum Toxin injections, often known as Botox, to help clients get the best possible outcomes from their treatment.

Botulinum Toxin can help reduce overactivity in specific muscles, but physiotherapy plays an important role in helping people improve movement control, posture, strength, mobility and function.

Physiotherapy management may include:

  • Assessment of neck movement, posture, muscle activity and functional goals
  • Education about dystonia and strategies to manage symptoms day to day
  • Gentle mobility and stretching exercises
  • Strengthening and motor control exercises
  • Postural retraining
  • Relaxation and breathing strategies
  • Advice around work, driving, sleep positioning and daily activities
  • Rehabilitation following Botulinum Toxin treatment to maximise benefit

The timing of physiotherapy around Botulinum Toxin treatment can be particularly valuable. Once overactive muscles begin to relax, there may be an opportunity to retrain movement patterns, strengthen underactive muscles and build confidence using new ranges of movement.

Amanda O’Neill from Coffs Neuro Physio says collaboration is key.

“Botulinum Toxin can be a really helpful part of cervical dystonia management, but it works best when it sits within a broader treatment plan,” Amanda says. “By working alongside Neurologists, we can help clients understand their movement patterns, build strength and control, and make the most of the window after injections. Having Dr Hugo Morales delivering Botox injections from our very own clinic means we can work even more closely together to ensure optimal management.”

Every person with cervical dystonia presents differently, so treatment needs to be individualised.

“Our role is to look at the whole person, not just the neck position,” Amanda says. “We want to understand how cervical dystonia is affecting their life, their confidence and their daily activities, then tailor physiotherapy around what matters most to them.”

For people living with cervical dystonia, support can help reduce the impact of symptoms and improve quality of life.
